    Use this form to complete all Equip Practicum registrations for all students in a single semester. You must attach your proposed syllabus for each practicum to this form, as well as any necessary documentation for new or existing Equip Instructors to be reviewed and receive approval. Please note, if you are teaching the same course to both college and seminary students, the course requirements are different. Keep in mind the registration deadlines. While our office may accept applications up to the add/drop deadlines, the student will be responsible to pay any late registration fees due to a late application.   • Course Information

    All Equip Practicum courses and updated syllabus templates are available on the Equip Network website. Contact the Equip office at equip@sebts.edu if you are unable to find the most updated templates.
  • Do we have an updated Resume or W9 for you?

    All new Equip Instructors must submit a resume and W9 with this registration form to receive approval and compensation. If returning Equip Instructors need to update their resume or W9, an updated one may be submitted via this form.

    As stated previously, graduate and undergraduate Equip courses have different course codes and requirements. Please list your graduate and undergraduate students separately as outlined below.
  • Graduate Students

    For each Equip Practicum course you are offering, add the course below and all the GRADUATE students who need to register for that course. If you are offering more than one course, select "add another course" to add the course and provide a list of students.
  • Undergraduate Students

    For each Equip Practicum course you are offering, add the course below and all the UNDERGRADUATE students who need to register for that course. If you are offering more than one course, select "add another course" to add the course and provide a list of students.
  • Equip Policy & Form Submission

  • In keeping with seminary policy, all Equip Leaders (Director, Mentors, and Instructors) are asked to affirm their agreement to honor and not instruct/mentor contrary to Southeastern Seminary’s Core Competencies and confessional statements—the Abstract of Principles and The Baptist Faith and Message (2000)—and two additional doctrinal statements—the Chicago Statement on Biblical Inerrancy and the Danvers Statement on Biblical Manhood and Womanhood.
